Case Notes with Purpose

Clear, Ethical Documentation for Support Workers

Not longer notes. Better notes.

Good case notes are not about using complicated language, writing down every detail or trying to make yourself sound professional.

They are about creating an accurate, respectful and useful record of someone else’s life and the support they received.

Case Notes with Purpose is a practical 10-part training series created specifically for Australian support workers. It helps workers understand not only how to write a case note, but how to make sound professional decisions about what to record, where information belongs, what action may be required and what the next authorised reader genuinely needs to know.

Why is this training different?

Case-note training is often reduced to a generic formula, rigid template or list of compliance rules. This course goes further.

Rather than teaching workers to write more, it develops the judgement needed to write information that is relevant, person-centred and useful.

The training is:

  • Created specifically for support workers - not adapted from nursing or allied health

  • Relevant across NDIS, aged care and broader community services

  • Informed by Australian legislation, standards, regulatory guidance and vocational training expectations

  • Written in practical, accessible language

  • Built around realistic support-work situations

  • Focused on dignity, rights, safety, continuity and accountability

  • Designed to build professional capability - not simply provide another template to copy

What will you learn?

Across the 10 modules, you will learn how to:

  • Understand why case notes matter and whose life the record is about

  • Recognise who may legitimately rely on, review or request access to documentation

  • Choose the right record for the right information

  • Distinguish between routine case notes, handovers, incident reports, medication records and other structured records

  • Respond to immediate needs before focusing on paperwork

  • Separate observable facts from assumptions, labels and unsupported conclusions

  • Accurately attribute information communicated by the person or provided by another authorised source

  • Describe support, participation, choices, relevant observations and outcomes clearly

  • Use respectful language without minimising risks or important information

  • Recognise when a case note is only one part of the required response

  • Record information that is relevant and proportionate without collecting unnecessary private details

  • Use practical tools to review a note before submitting it

  • Build more consistent, useful and professional documentation habits

More than a certificate

The certificate can help demonstrate that you have invested in your professional development - but the real value is what changes in practice.

The aim is for you to leave the course feeling more confident about:

  • What belongs in a case note

  • What belongs somewhere else

  • How to describe what actually happened

  • How to keep the person visible in the record

  • When documentation must be accompanied by action, reporting or escalation

  • Whether your note will genuinely help the next authorised reader

Because a case note is not just paperwork.

It is a record about someone else’s life - and the way we write matters.

Important course information

This is non-accredited foundation training and provides a certificate of completion. It does not constitute a formal workplace competency assessment, guarantee provider registration or replace nationally recognised training, legal advice, supervision, workplace induction or organisation-specific procedures.

Documentation and reporting requirements vary between services and situations. Learners must continue to follow the person’s current plan, their role and level of competence, authorised workplace systems, applicable legislation and their organisation’s policies and reporting pathways.
